Hearing & ENTA blocked nose, a heavy head and a second week of sniffing, is it still that cold, or has it quietly turned into a sinus infection? This is one of the most common questions we hear in the ENT room, and the honest answer is that the two can look almost identical in the first few days.
The real difference lies in how the illness behaves over time. A cold peaks early and fades on its own. Sinusitis lingers past the ten-day mark, or seems to improve and then suddenly turns worse again. Here is how to tell the two apart, and what genuinely helps a sinus infection clear.
A common cold is a viral infection. It typically builds over two to three days, sneezing, a watery running nose, a mild sore throat, sometimes a low fever, and then settles gradually within seven to ten days. The discharge often thickens and turns yellowish towards the end. That colour change alone does not mean you need antibiotics; it is a normal part of recovery.
Children may take a little longer to recover, and a dry cough can trail on for two or even three weeks while the airways settle. The key idea is trajectory: with a cold, each day past the peak should feel a little better than the one before, even if progress is slow.
The sinuses are air-filled spaces in the bones of the cheeks and forehead. When a cold inflames their lining, mucus can get trapped inside instead of draining out, and stagnant mucus can become infected. Suspect sinusitis when symptoms cross ten days without improving, or when you begin to recover and then worsen again with fresh fever and facial pain, doctors call this pattern double worsening.
Typical sinusitis complaints include pressure or pain over the cheeks, forehead or around the eyes that feels worse on bending forward, thick yellow-green discharge, mucus trickling down the back of the throat, a reduced sense of smell, and sometimes an ache in the upper teeth or a blocked feeling in the ears.
Most sinus infections begin as viral infections and settle with supportive care. Saline nasal rinses or sprays help wash out thick mucus, steam inhalation can ease the sense of pressure, and plenty of fluids keep secretions thin so they drain more easily. Paracetamol can be used for pain and fever as advised. Medicated decongestant sprays should not be used beyond a few days, because overuse causes a rebound blockage that is worse than the original problem.
Antibiotics are not a routine fix. They help only when the doctor suspects a bacterial infection, judged by how long and how severely you have been unwell and what the examination shows. Taking an antibiotic for every cold does not speed up recovery, and it makes future infections harder to treat.
If you get several episodes a year, something may be blocking normal sinus drainage, a deviated septum, nasal polyps, swollen turbinates or long-standing allergy, which is a very common thread we see in Pune through the changing seasons.
A nasal endoscopy, a quick camera examination done right in the clinic, lets the ENT doctor see these drainage pathways directly and find out what keeps the cycle going. Treating that root cause matters more than treating each episode: allergy control, correctly used nasal sprays, and only where a structural blockage truly needs correction, a procedure planned at an accredited partner hospital. The clinic itself always treats medically first.
Use this checklist when you are stuck at the 'is it still a cold?' stage, and keep the red flags below in mind. Complications of sinusitis are uncommon, but when they occur they move quickly and need urgent attention rather than watchful waiting.
