Hoarse Voice That Won't Settle? Voice Strain, Voice Therapy and When to See an ENT

A great many people in Pune use their voice as a work tool: teachers, lecturers, call-centre and customer-support teams, sales staff, trainers, lawyers, singers and anyone who spends the day on video calls. For them, a voice that turns rough, weak or tired by evening is more than an annoyance. It affects work, confidence and income.

Most hoarseness is short-lived and harmless, often following a cold or a long day of talking. But a voice that stays hoarse deserves attention, both because voice strain can be treated well and because, occasionally, persistent hoarseness is the first sign of something that needs early care.

How your voice works, in brief

Your voice is produced by two small folds of tissue in the voice box (larynx) that vibrate as air passes between them. Anything that makes those folds swollen, stiff, uneven or unable to close properly changes the sound. That is what we hear as hoarseness, breathiness or a voice that tires quickly.

Common causes of a hoarse voice

  • Viral laryngitis with a cold or throat infection, usually better within one to two weeks
  • Voice overuse and strain: talking loudly for long hours, over background noise, or with poor breath support
  • Acid reflux reaching the throat, which can cause hoarseness, throat clearing and a lump-in-the-throat feeling, sometimes without heartburn
  • Vocal nodules or polyps: small, benign swellings on the vocal folds, common in people who use their voice heavily
  • Smoking and vaping, which irritate and dry the vocal folds
  • Allergy and post-nasal drip, which lead to constant throat clearing
  • Less commonly, nerve problems affecting vocal fold movement, thyroid conditions, or growths on the vocal folds

When hoarseness needs an ENT examination

See an ENT specialist if your voice has been hoarse for more than three weeks, and sooner if you smoke or used to smoke. The doctor examines the throat and voice box, and if a closer look at the vocal folds is needed, a scope examination is arranged. It is quick, done with local anaesthetic spray, and it is the only way to see exactly what is happening to the folds.

What voice therapy is

Voice therapy is treatment with a speech-language therapist. It is not about learning to speak differently in a dramatic way. It teaches you to produce your normal voice with less effort and less strain on the vocal folds. For voice strain and for many vocal nodules, therapy is the main treatment, and surgery is considered only if therapy has not helped enough.

A typical programme includes:

  • Breathing support, so the voice is powered from the breath rather than squeezed from the throat
  • Resonance exercises that let the voice carry without shouting
  • Relaxation of the neck, jaw and shoulder muscles that tighten under stress
  • Practical changes for your job, such as microphone use, pacing and planned voice breaks
  • Home exercises, practised for a few minutes daily between sessions

Voice care you can start today

  • Sip water through the day. Well-hydrated vocal folds vibrate more easily.
  • Avoid shouting over noise. Move closer, use gestures, or use a microphone for classes and presentations.
  • Do not whisper to rest your voice. Whispering can strain the folds more than a soft, easy voice.
  • Replace throat clearing with a sip of water or a gentle swallow.
  • Take short voice breaks during long calls or teaching days.
  • If you have reflux, avoid large late meals and lying down soon after eating.
  • Stop smoking. Your voice is one of many reasons to.
