Understanding It

What is a vertigo test?

A vertigo test is not a single test but a set of checks that look at how well your balance system is working. Your sense of balance depends on three things working together: your inner ear, your eyes, and the position sensors in your joints and muscles. When one of them sends the wrong signal, the result is vertigo, that unsettling feeling that you or the room is spinning. A vertigo evaluation works out which part is at fault.

The single most useful thing a proper evaluation does is separate an inner-ear cause from a brain or nerve cause. The large majority of dizziness comes from the inner ear and is very treatable, often quickly. A smaller number of cases point to something that needs different, sometimes urgent, attention. Knowing which is which is the whole point, and it is why guessing from symptoms alone is a poor idea.

When a vertigo evaluation helps

Detailed history

We ask exactly how the dizziness feels, what sets it off, how long it lasts and what comes with it. This alone points strongly toward the cause.

Clinical examination

Focused bedside tests such as positional testing (the Dix-Hallpike manoeuvre) and a head impulse test check your inner-ear balance reflexes and help identify BPPV.

Hearing assessment

Because the inner ear controls both balance and hearing, a hearing test is often part of the picture, especially when there is ear fullness or ringing.

Advanced tests in our lab

When the cause is unclear, we run instrumented vestibular tests in our in-house lab, and arrange an MRI only if something beyond the inner ear is suspected, guiding you through what each result means.

The tests that may be part of your evaluation

Positional testing (Dix-Hallpike)

Gentle, guided head and body movements that reproduce and identify BPPV, the commonest cause of vertigo, and show which ear and canal is involved.

Head impulse test

A quick check of the reflex that keeps your vision steady when your head moves, which helps separate an inner-ear cause from a central one.

Hearing test (audiometry)

Measures hearing across frequencies, important when vertigo comes with ear fullness, ringing or fluctuating hearing, as in Meniere-type problems.

Videonystagmography (VNG)

An instrumented test that records eye movements to map inner-ear function and pinpoint the affected side, done here in our in-house vestibular lab when a detailed balance map is needed.

Video head impulse test (vHIT) and VEMP

Advanced tests that assess individual balance canals and the otolith organs, useful in conditions like vestibular neuritis and Meniere's disease, and available as part of our in-house vestibular testing.

Balance and posturography assessment

Looks at how your eyes, inner ear and body work together to keep you steady, and helps judge fall risk, particularly in older adults.
