We also do not have an MD allergist or a clinical immunologist on site. If your plan needs any of those, you will need a referral to a specialist allergy centre, and we will point you in the right direction rather than pretend otherwise.

Immunotherapy is a long course of treatment that slowly exposes your immune system to tiny, controlled amounts of the very thing you react to, such as dust mite or certain pollens. The idea is to gently teach the body to stop overreacting, over months and years. It is given either as a series of injections or as drops or tablets under the tongue. It is not a quick fix and it is not a trick. It is a slow programme that has to be started and supervised by a doctor trained for it, because reactions can happen along the way.

It is not for everyone. Broadly, it tends to be considered for:

• People whose allergic rhinitis is clearly driven by one or two confirmed triggers that they simply cannot avoid
• People who still struggle badly despite sensible avoidance and steady medical management
• People whose specialist has weighed the commitment of a multi year course against the small risk of reactions

That is exactly why we read the report alongside your history and examination, rather than handing you a printout and calling it an answer.

If immunotherapy is right for you

Allergy is usually a nuisance rather than a danger, but a severe allergic reaction, called anaphylaxis, is a medical emergency. Call 108 or go straight to the nearest emergency department if you or someone with you has any of these:

• Difficulty breathing or a sudden wheeze
• Swelling of the throat, tongue, or lips
• A tight throat, or a hoarse or choked sounding voice
• Feeling faint, collapsing, or losing consciousness
• A widespread rash together with any of the above

Please do not try to wait it out, and do not rely on an antihistamine tablet in this situation. Antihistamines do not treat anaphylaxis. Emergency care does. If you already carry an emergency injector that a doctor prescribed for a known severe allergy, use it exactly as you were taught and call 108 at once.
