Low Testosterone in Men: The Signs, the Causes, and When to Get Checked

If you have been feeling flat lately, low on energy, less interested in sex, and just not yourself, and a friend or an online quiz has told you it must be low testosterone, I understand why that worries you. Men come to my clinic in Viman Nagar with exactly this fear, often after reading something at 1 in the morning and quietly panicking about it for weeks.

Here is the honest picture. Low testosterone is real, it is common enough that it is worth taking seriously, but the signs are vague and overlap with many other things. So please do not diagnose yourself, and please do not start any hormone or supplement on your own. Let me walk you through what these signs mean, what can cause them, and how we actually check.

What testosterone does, in plain terms

Testosterone is the main male hormone. It has a hand in your sex drive, your erections, your muscle and bone strength, your mood and drive, and even how you sleep. When the level drops meaningfully, some of these can dip together. The catch is that every one of them can also dip for reasons that have nothing to do with hormones, which is why the signs alone never settle the question.

The signs men usually notice

These are the common ones people describe to me. You do not need all of them, and having a few does not confirm anything on its own.

  • Lower interest in sex than you used to have
  • Tiredness that a good night of sleep does not fix
  • Low mood, low motivation, or a short fuse
  • Changes in erections or morning erections
  • Feeling that your muscle strength has slipped, or gaining weight around the middle
  • Trouble concentrating or a general foggy feeling

Why these signs are tricky

Look at that list again and you will see the problem. Tiredness, low mood and low sex drive are some of the most non-specific complaints in all of medicine. The very same signs show up in men with completely normal testosterone. That is the whole reason we test rather than assume.

What can actually lower testosterone

When the level genuinely is low, there is usually a reason behind it, and finding that reason matters as much as the number itself.

  • Age, since levels tend to drift down slowly as men get older
  • Carrying extra weight, which is one of the most common and most reversible causes
  • Type 2 diabetes and related metabolic problems
  • Poor or broken sleep, including untreated sleep apnoea
  • Thyroid and other hormone gland issues
  • Long standing stress, heavy alcohol use, and some long term medicines

Notice how many of these are things we can improve. That is often the good news hiding inside a worrying symptom.

How we actually diagnose it

There is no shortcut and no reliable home guess. Testosterone is checked with a blood test, and timing matters, so it is usually taken in the morning when levels are at their highest. Because a single reading can be misleading, a low result is normally confirmed with a repeat test rather than acted on straight away.

Just as important is the conversation and examination that goes with it. In the clinic we go through your symptoms, your sleep, your weight, your other health conditions and your medicines, and we often check related things like blood sugar, thyroid and other hormones. The number only means something when it is read alongside all of that. Treatment, if any is needed, is then built around you and your cause, not copied from someone else. Outcomes vary from person to person, and any real improvement follows the right diagnosis and the right care, not a quick fix.

Please do not self-medicate

This is the part I feel strongest about. Please do not buy over the counter testosterone gels, injections, or boosting supplements off the internet or a gym counter, and please do not borrow a friend's prescription. Taken without proper testing and supervision, testosterone can do real harm, including affecting your own natural production and your fertility, and many so called booster supplements are unregulated and unclear about what is even inside them. Any hormone treatment belongs with your treating doctor, who can decide whether you need it at all, monitor you safely, and stop or adjust it.

When to see a doctor

Book a proper review if these signs have lasted several weeks, are affecting your work, your relationship or your sleep, or if you already have diabetes, obesity or thyroid issues. There is nothing awkward about it from our side. It is a routine, private conversation and a simple blood test.
