Most people use the words 'cold' and 'flu' interchangeably, but they are caused by different viruses and have meaningfully different courses. Knowing which one you have helps you judge how much rest you need, when you might be contagious, and crucially, when to seek medical attention rather than waiting it out.
What is the main difference between a cold and the flu?
| Feature | Common Cold | Influenza (Flu) |
|---|---|---|
| Onset | Gradual, over 1 to 2 days | Sudden, within hours |
| Fever | Rare or mild (under 38.5C) | Common, often 38.5 to 40C |
| Body aches | Mild | Severe, often described as feeling hit by a truck |
| Headache | Mild | Pronounced |
| Fatigue | Mild to moderate | Severe, can last weeks |
| Runny or blocked nose | Very common | Sometimes present |
| Sore throat | Common | Sometimes present |
| Cough | Mild to moderate | Common, can be severe |
| Chills | Uncommon | Common |
How long does each illness last?
A cold typically peaks around day 2 to 3 and resolves within 7 to 10 days. Lingering nasal congestion may continue for up to two weeks. Influenza tends to be more intense: the acute phase lasts 5 to 7 days but fatigue and weakness can persist for several weeks, particularly in older adults, pregnant women, and those with underlying conditions.
When should you see a doctor?
- Fever above 39 degrees Celsius that does not respond to paracetamol or lasts more than 3 days
- Difficulty breathing or shortness of breath
- Chest pain or pressure
- Severe or worsening headache
- Confusion or altered consciousness
- Signs of dehydration: no urination for 8 hours, very dark urine, dizziness on standing
- Symptoms that improve then suddenly worsen (possible secondary bacterial infection)
- Any flu symptoms in a pregnant woman, elderly person, young child under 2, or someone with diabetes, heart disease, or lung disease
Does a cold or flu need antibiotics?
No. Both colds and influenza are caused by viruses, and antibiotics have no effect on viruses. Taking antibiotics unnecessarily contributes to antibiotic resistance without helping you recover faster. Antibiotics are only warranted if a bacterial complication develops, such as bacterial sinusitis, ear infection, or pneumonia, and your doctor has confirmed this.
How do you recover faster from a cold or flu?
- Rest as much as possible, especially in the first two to three days
- Stay well hydrated with water, warm fluids, and soups
- Use steam inhalation for nasal congestion
- Paracetamol (as directed for your weight and age) for fever and body aches
- Avoid close contact with vulnerable people while you are symptomatic
- Do not smoke, as it prolongs respiratory illness significantly
Is there a vaccine for the flu?
Yes. The influenza vaccine is updated each year to match circulating strains and is recommended for elderly individuals, young children, pregnant women, people with chronic health conditions, and healthcare workers. It does not prevent every flu but significantly reduces severity and the risk of hospitalisation.
