Some of those exercises are genuinely useful, we teach several of them in clinic ourselves. Others can make you feel worse, and doing the right exercise for the wrong condition simply wastes weeks.
This guide explains what Brandt-Daroff exercises actually are, which balance practices are reasonably safe to do at home, and where a proper diagnosis still has to come first.
Your sense of balance is teamwork between three systems, the inner ear, the eyes, and the position sensors in your muscles and joints. When the inner ear is disturbed, the brain can gradually learn to lean more intelligently on the other two and to filter out faulty signals. This relearning is called vestibular compensation, and exercises are simply structured practice that encourages it. The catch: the brain only relearns what you make it rehearse, which is why the choice of exercise depends on the diagnosis, not on which video played first.
Brandt-Daroff exercises are a classic home routine most often suggested for BPPV, for example, for mild leftover dizziness after a repositioning procedure such as the Epley manoeuvre, or occasionally while waiting for a proper assessment. From sitting on the edge of the bed, you lie down briskly onto one side with your nose angled about 45 degrees upward, stay until the dizziness fades, usually around thirty seconds, then sit up, pause, and repeat on the opposite side. A common prescription is about five repetitions per side, two to three times a day, but your doctor will personalise the number and the duration.
Expect them to provoke brief dizziness; that is partly how they work. What they should not do is trigger hour-long spinning, repeated vomiting or completely new symptoms. And a point of honesty: for confirmed BPPV, a correctly performed Epley manoeuvre in clinic usually settles matters faster than weeks of Brandt-Daroff repetitions, the exercises are a companion, not always the main treatment.
Alongside or after treatment, most people can safely practise gentle balance work at home, provided a wall, a sturdy chair or a family member is within reach for the first few sessions. Sensible options include:
Some things genuinely belong in a clinic. Top of the list is a self-administered Epley manoeuvre copied from a video: the procedure must target the correct ear and the correct canal, and doing it on the wrong side can occasionally push crystals into another canal and make the vertigo more complicated. Exercises are also not a substitute for diagnosis when symptoms are new, severe or changing. Best avoided:
With the right diagnosis and consistent practice, many people notice steadier days within a few weeks, though the pace varies with age, the underlying cause and how regularly the exercises are actually done. If there is no improvement after a fair trial, that is not a failure on your part, it is a signal to re-examine the diagnosis, and it deserves a review visit rather than months of silent struggling.
Brandt-Daroff exercises are designed around BPPV-type positional dizziness. They do little for vestibular migraine, inner-ear inflammation or blood-pressure-related giddiness, each of those needs a different plan. A short examination, sometimes with a hearing test, usually sorts this out. And one non-negotiable rule: if dizziness ever arrives suddenly together with slurred speech, a drooping face, double vision, weakness of an arm or leg, or the worst headache of your life, treat it as a possible stroke, call 108 or go straight to the nearest emergency department. No home exercise applies in that situation.
