Sciatica and Slipped Disc: When Leg Pain From Your Back Is Serious

You bend to pick something up, or you get off your two-wheeler after a long ride on New Airport Road, and a sharp pain shoots from your lower back down the back of your leg. Sometimes it reaches the calf or the foot. It can be frightening, and it makes simple things like sitting through an office shift or climbing your stairs feel like a real struggle.

The good news, which I share with many worried patients in my clinic, is that most of this pain settles with the right care and a little time. Let me explain in plain words what is happening, and, just as important, the few warning signs that mean you should not wait at all.

What sciatica actually is

Sciatica is not a disease by itself. It is the name we give to pain that travels along the sciatic nerve, the large nerve that runs from your lower back, through the hip and down each leg. When something presses on or irritates that nerve near the spine, you feel it down the leg, most often on one side.

The common reason is a slipped disc (doctors also call it a herniated disc). Between the bones of your spine sit soft cushions called discs. When part of a disc bulges out, it can press on a nearby nerve root. Age, long hours of sitting, heavy or awkward lifting, and sometimes plain bad luck can all play a part.

How it usually feels

  • Pain that starts in the lower back or buttock and shoots down one leg, sometimes as far as the calf or foot
  • A burning, electric or shooting quality, rather than a simple dull ache
  • Tingling, or a pins and needles feeling, in the leg or foot
  • Numbness in part of the leg
  • Pain that gets worse when you sit for long, cough, sneeze or strain

Usually it affects one leg. Interestingly, the back pain may even be mild while the leg pain is the loud complaint that brings you to see me.

The reassuring part: most cases improve

Here is what I tell people first. In most cases, sciatica from a slipped disc calms down over a few weeks with simple, conservative care. The disc settles, the inflammation around the nerve reduces, and the leg pain slowly fades. You do not rush to scans or to surgery for ordinary sciatica.

  • Keep gently active as your pain allows. Long bed rest usually makes the stiffness worse, not better.
  • Short walks, changing your position often, and breaking up long unbroken sitting all help.
  • Mind your posture at your desk and on your two-wheeler, and support your lower back.
  • Physiotherapy, guided by a trained professional, is one of the most useful steps for many people.
  • Give it time. Recovery here is gradual, not overnight, and that is normal.

I will not name specific medicines here, because pain relief has to be matched to you by your own doctor. Please do not keep self medicating for long stretches without an examination.

Red flags: when sciatica is an emergency

A small number of cases are different and need urgent attention. If the nerves at the very bottom of the spine get badly squeezed, you can lose important functions. Do not watch and wait if you notice any of these signs.

The signs in the right column are emergencies. Call 108 or go straight to the nearest emergency hospital. Caught early, these problems can often be helped. Left too long, the nerve damage can become permanent, so please do not delay.

When to see a doctor (without an emergency)

Even without red flags, come and see a doctor if the leg pain is severe, if it is not improving after a couple of weeks of sensible care, if it keeps coming back, or if it is stopping you from working or sleeping. A proper examination works out which nerve is involved and rules out other causes.

Scans like an MRI are not needed for everyone. We usually reserve imaging for cases that are not improving, that show red flags, or where we are planning a specific treatment. Surgery is considered only for particular situations, and that decision is made carefully by the treating doctor after weighing your examination and your scans. It is never the first step.
