Gallstones: Symptoms and When the Gallbladder Needs to Come Out

If a scan has just told you that you have gallstones, it is normal to feel a little shaken. The word stones sounds alarming, and most of us picture surgery straight away. Take a breath. In real life, gallstones are very common, and a large number of them sit quietly and never trouble you at all.

I am Dr. Rahul Wagh, and I meet worried patients about this almost every week at our Viman Nagar clinic. Some have had one bad night of pain after a heavy dinner. Others found their stones by accident during a scan for something else. The right next step is different for each person, so let me walk you through it in plain language.

What gallstones actually are

Your gallbladder is a small pouch that sits under the liver on the right side of your belly. It stores bile, a fluid that helps your body digest fatty food. Sometimes the bile becomes concentrated and forms small solid pieces, and these are gallstones. They can be as tiny as a grain of sand or as large as a small marble, and you can have one or many.

Certain things make stones more likely, such as being female, family history, being overweight, rapid weight loss and skipping meals for long hours. Many people in Pune have a busy, meal-skipping routine because of office shifts and long commutes, and that pattern does not help. Still, plenty of people with none of these risks get stones, so please do not blame yourself.

Many gallstones are silent and need no treatment

This is the part that surprises most people. If your stones are not causing symptoms, in most cases they simply need watching, not surgery. We call these silent stones. There is no benefit in rushing to remove a gallbladder that is behaving itself, and doing nothing is often the correct medical choice. What matters is that a doctor confirms the stones are truly silent and keeps an eye on things over time.

Typical symptoms to notice

When gallstones do cause trouble, the pattern is usually recognisable once you know it. The classic complaint is pain in the upper-right part of the belly, often starting after a fatty or heavy meal. Common symptoms include the following.

  • A dull or gripping pain in the upper-right abdomen, sometimes spreading to the back or the right shoulder blade.
  • Pain that tends to come on after oily food, a wedding feast or a rich restaurant meal.
  • Nausea or a feeling of wanting to vomit along with the pain.
  • A bloated, heavy feeling and burping after eating, which many people mistake for ordinary gas or acidity.

This kind of pain often settles after some time, then returns another day. If you keep getting these attacks, it is worth getting evaluated rather than living from one painful night to the next.

Red flags: when to seek urgent care

Some gallstone problems are genuine emergencies, usually because a stone has blocked a duct or an infection has set in. Please do not wait these out at home. If you notice any of the following, treat it as urgent.

  • Severe pain in the upper-right belly that does not ease and keeps you doubled over.
  • Fever with chills or shivering along with the belly pain.
  • Yellowing of the eyes or skin, or unusually dark urine, which can point to a blockage.
  • Repeated vomiting so that you cannot keep any fluids down.

For any of these, call 108 or go straight to the nearest emergency hospital. Do not ride out to us on a two-wheeler in that state, and do not wait for clinic hours. A blocked or infected gallbladder needs hospital care quickly, and getting there fast is the safest thing you can do.

When the gallbladder needs to come out

When stones keep causing pain, or when there has been a complication, removing the gallbladder is often the right and lasting answer. The good news is that this is one of the most common and well-established operations done today. It is usually performed by keyhole (laparoscopic) surgery through a few small cuts, and the body manages perfectly well without the gallbladder afterwards. Recovery is generally quicker than most people expect, though every case is individual and your surgeon will guide you honestly on what to expect.
